MEDICAL RELEASE & CONSENT
Bend Unleashed has full medical release to make all decisions while the owner’s dog is in the care of Bend Unleashed. Bend Unleashed has authorization to make medical decisions in the event of injury or illness on the owners’ behalf. Bend Unleashed has my dogs’ best interest in mind and we trust and support their judgement to keep our dog safe and cared for. In the event of an emergency or illness, they may make all medical decisions. I (owner) understand I will be financially responsible.

VACCINATIONS
I understand it is my responsibility to keep my dog fully vaccinated and to provide Bend Unleashed with current records. I understand if my dogs do not have all the required vaccines, we will be turned away and I will be held responsible for any charges. I understand I may only bring my dog to Bend Unleashed if they are healthy and clean (no skunk odor, feces, lice, fleas, etc…).

CANCELLATIONS
I understand Bend Unleashed has a minimum of a 3-day (72 hours) cancellation period for non-holiday / peak season boardings, 14-day cancellation period for holiday and peak season boardings. Daystay and Grooming requires 24-hour notice.

CHECK IN / CHECK OUT
I understand boarding check-in is between 3:00pm to 5:30pm and check-out is from 8:30am to noon. We understand early drop-offs and late pick-ups may be pre-arranged based on availability for an additional fee. I understand an additional late pick up fee will be added if I arrive after closing if the office is still available, otherwise I understand I may need to wait until the following business day. I understand Bend Unleashed office is closed on Holidays and Sundays.

BARKING AND BEHAVIOR
I understand that excessive barking may cause anxiety and undo stress to other dogs as well as my own. Bend Unleashed does everything to calmly detour excessive barking. In the event a dog will not stop barking, BU has permission to use barking deterrents as needed. This includes citronella, vibration, and static collars. Bend Unleashed will do everything in their power to keep my dog in appropriate social groups. In the event behavioral issues arise, Bend Unleashed may need to switch reservation accommodations which may include Private Plays for an additional charge.

FACILITY
While on Bend Unleashed property I will walk my dog in and out of the facility on a leash for their safety. I understand that even though it is fun and tempting to watch my dog play, I will refrain from going near the fences so it will not upset the other dogs which could lead to safety issues.

DAMAGE OF PROPERTY
I am responsible for all damages and/or fees that may occur to my dog while in the care of Bend Unleashed. I am aware if any charges occur, I will be responsible for paying for them at the time of check out.

GROOMING RELEASE
Bend Unleashed will not be held responsible for any injuries that may be obtained during grooming procedures. Bend Unleashed takes every effort to keep your dog safe, but a dog’s unpredictability may be an issue during grooming procedures.

ABUSE AND NEGLECT
Bend Unleashed has the moral responsibility to be an advocate for the dogs in their care. If they feel at any time a dog is unnecessarily suffering, Bend Unleashed will contact proper authorities at the owner’s expense.
